What Happens After You Refer

Referral Received

We receive your referral and contact the patient promptly to schedule their initial evaluation — typically within a few days.

Comprehensive Evaluation

The patient sees our sports medicine physician for a thorough history, physical exam, and — where indicated — imaging review or ordering. Enough time is taken to actually understand the problem.

Coordinated Care Plan

The physician coordinates with our rehab and manual therapy team to build a single, integrated plan. The patient knows what the plan is, why, and what success looks like.

Communication & Discharge

You receive documentation after the initial visit. We communicate throughout care when clinically relevant and send a discharge summary upon completion.

What You Get Back

We Are Not a Black Box

One of the most common frustrations for referring providers is sending a patient to a specialist and hearing nothing back. We take a different approach.

You will know what we found, what we recommended, and how the patient is progressing. If something comes up that changes the clinical picture — positive or negative — we communicate it. We are a partner in your patient's care, not a destination.

Initial Evaluation Report

A clear summary of our clinical findings, working diagnosis, and treatment plan — sent to your office after the initial visit.

Ongoing Communication

If the clinical picture changes — imaging reveals something unexpected, conservative care isn't progressing, or surgical consultation becomes relevant — we reach out.

Discharge Summary

At the conclusion of care, you receive a summary of what was done, functional outcomes achieved, and any ongoing recommendations for the patient's long-term health.
